Your hospital must be in located in the District of Columbia, Georgia, or New Jersey in order to apply to the multi-hospital learning collaborative.

November 2013

Baseline survey available for all District of Columbia, Georgia, and New Jersey hospitals (contact to obtain the survey for your facility).  Please include your hospital name and address in the email that you send to our customer service department.
