Complete the Baseline Survey

Complete the baseline survey to receive a free copy of AWHONN’s Obstetric Hemorrhage Monograph. Also, by completing this survey, your hospital will provide AWHONN with valuable data on postpartum hemorrhage practices, which will help shape future AWHONN work in postpartum hemorrhage. All data will be kept confidential and AWHONN will only discuss the results of the survey in the aggregate.

Email customerservice@awhonn.org if your hospital is in the District of Columbia, Georgia, or New Jersey and you want to receive a copy of the survey for your hospital.  Please include your hospital name and address in the email that you send to our customer service department.
